OpenApply can assist schools with converting existing forms into online formats within the system. To facilitate this process, please provide files in Word format, copyable PDFs, or other online form formats.
We can customize the forms based on the files you provide. For a clearer understanding of what can be built, please refer to the table below, which includes a general list of forms and relevant examples.
Please ensure that you provide copyable Word or PDF versions of your forms for accurate copying and pasting during the form-building process. If you wish for the forms to support multiple languages, you may also include translated versions of the forms.
Additionally, please note that forms can be updated or modified at a later date as needed.

Registration of Interest
A short and simple enquiry form
Only can have one for each school
Available to enable/disable to show on landing page as a button
Application
Official & complete form for application
Can have multiple application forms for different campuses or programmes
Available to enable/disable to show on landing page as a button
Supplementary
Build any forms into a supplementary form
For example: medical form, bus application, scholarship, boarding, etc
Supplementary forms would be linked to checklist items for parents to fill in
Questionnaire (Recommendation)
Confidential online recommendation form for student's previous/current school's teacher/principal to fill in
The parent completes the first part of the form, which they access via their checklist, entering the name, e-mail address and school of the teacher. The parent clicks ‘Save & Send via Email’, which sends an e-mail request directly to the teacher.
The teacher will receive an email with a unique link, which takes them directly to the online form for that particular student. The teacher completes the form online. You receive the response electronically, and can view the data in OpenApply which is confidential to the parents.
Office Use Forms
It is possible to set a supplementary form as office use only to record any confidential or internal data, for example entrance exam results. Admin can view the form by clicking ‘More’ button at the student’s profile
